Applications
After you have reviewed the programs offered by Service Dogs of Virginia, and the criteria we use to make placement decisions, you may elect to apply for a dog. You may do so by filling out our online application or printing an application and mailing it. There is a $25 application fee.

**Please Note: Service Dogs of Virginia only places service dogs with clients who reside in the Commonwealth of Virginia.**

Apply for an Autism Dog
- We are not accepting applications for Autism Service Dogs at this time, as our current waiting list exceeds two years. (modified 9/5/12)
